Pragma Health enables GDPR-compliant digital questionnaires for therapy practices. Patients fill out questionnaires via QR code — no app, no account needed.

Access

Log in with your regular Pragma credentials.

1. Create a questionnaire

1. Click + New Questionnaire
2. Choose a template (PHQ-9, GAD-7, WHO-5, Initial Anamnesis) or start blank
3. Questions are pre-filled automatically — adjust as needed
4. Enter a title → Create questionnaire

2. Add a patient

1. Click Add patient
2. Name (real name or anonymous code like P-2026-001)
3. Optional: Internal ID for your patient records

GDPR tip

Use anonymous codes instead of real names for maximum GDPR compliance. You can map codes to your internal records.

3. Generate a QR code

1. In the dashboard: click QR ▶ next to a questionnaire
2. Optionally assign a patient
3. Generate QR code → page with QR code and link opens
4. Print the QR code or send via WhatsApp/email

4. Patient fills out the form

The patient opens the link or scans the QR code:

• No app download needed
• No account, no password
• Form opens directly in the browser
• After submitting: score appears instantly in the dashboard

5. Evaluate scores

In the patient profile (/health/patients/{id}):

• Score timeline
• Color coding: 🟢 ≤5 · 🟠 ≤15 · 🔴 >15
• First vs. last score comparison
• All sessions with date

Upload documents

Dashboard → Upload document:

• Formats: PDF, JPG/PNG, Word (max. 10 MB)
• Categories: Anamnesis, Finding, Report, Consent, Other
• Optional: assign to a patient
